    Notes

The Utah State Archives hold Utah government records that have historical value, such as annual reports, legislative bills and correspondence.

Their holdings relating to leprosy are as follows:

From the Davis County (Utah) Health Dept Nursing Division (Series #10787) and the Southwest Utah Public Health Department (Utah) (Series #16560), there are files on Leprosy Surveillance investigations. Access to these is restricted.

There are also case file indexes (Series #82998 - 1965-[ongoing]; Series #82996 – [ca. 1936]-[ongoing]) from the Utah Bureau of Epidemiology. Information in these files includes patient case number, name, address, race, sex and age; physician and hospital with which the case is affiliated.

Leprosy Surveillance records also show information such as whether the patient ever touched armadillos and all known suspected patients with leprosy who have had contact with the patient.
